- Act 34 – PA State Criminal Clearance (instructions)
- Act 151 – Child Abuse Clearance (instructions)
- Act 114 – FBI Clearance(instructions)
- Service Code: 1KG6XN
- PDE-6004 – Arrest/Conviction Report and Certification Form
- Act 168 – Sexual Misconduct/Abuse Disclosure Release Form
- You must complete one Act 168 form for each place you have worked, in your lifetime, where you had direct contact with kids and you were paid. If employed currently, we will also need one completed for your current employer.
- Act 126 – Child Abuse Recognition and Reporting Certification

- Physical/TB Test
- Payroll Packet – Will be provided upon hire.
